Templates & Worksheets
People come to this page looking for a form. What the firm publishes instead is a set of private worksheets and printable checklists that help you inventory what you have and what is at risk. Letters that carry legal weight are sent by the firm, not filled in from a template.
By the document people ask for
The Worksheets
Preservation letter (vehicle data)
What a vehicle’s black box and connected services record, and a worksheet to note which vehicles and accounts hold it. The firm sends the preservation letter.
Open the worksheet →Camera footage request
Where crash-scene video tends to exist and how quickly it is overwritten, with a worksheet to record which cameras you saw. The firm sends the request.
Open the worksheet →Evidence file
A private checklist of what you already have, what still needs gathering, and which items are perishable. It stays on your device.
Open the organizer →ER bill review
A guided pass through the line items that most often turn out to be errors on an emergency-room bill, and how to raise a question with the billing office.
Check your bill →Printable checklists
For the glovebox and the hospital bag: trauma intake, ER billing, school bus stops, and a lease you are about to sign, on paper.

Why There Is No Demand-Letter Template
A demand letter is built from one case’s medical record, one case’s liability evidence, and one case’s coverage picture, and it is sent when the medical story is stable enough to price. A fill-in version gets the structure right and everything else wrong, and adjusters can tell. I draft each demand from the file, with the documentation attached, so the firm does not publish one. If you are at the point of writing a demand, you are at the point where a conversation costs nothing.
Important Information: These worksheets are general information and are not legal advice. They run in your browser and send nothing to the firm. Using a worksheet or contacting Andrade Law does not create an attorney-client relationship, and nothing sent through this website is confidential or privileged unless the firm agrees to represent you in writing. Every case depends on its facts, and past results do not guarantee future outcomes.
